8/3/2023 EnchantaVaughn Approved Behavior: Ashiyah initially presented with talkative behaviors as she probed the QMHP-C and asked different questions. Ashiyah later displayed quiet behaviors as she shrugged her shoulders and did not want to answer QMHP-C questions. Ashiyah did not respond as information was shared on anxiety and stress. Ashiyah was assisted in practicing general communications skills. Behavior: Ashiyah understood effective problem-solving skills to help solve or overcome a problem or stressor the she is currently facing. Ashiyah understood that when she becomes agitated she needs to refrain from taking it out on people and to communicate how she is feeling to others. Behavior: Ashiyah participated in reviewing key concepts needed to relax in order to continue to grow and mature into a young lady. Ashiyah showed good understanding of what it means to be respectful, encouraging, mindful, and fair to others. Ashiyah is observed correcting her mistakes and making good choices. Ashiyah was able to utilize coping skills to eliminate stressors and unwanted feelings. Intervention: QMHP-C offered new ways to heighten Ashiyah awareness of her ill-mannered behaviors, as well as helping her with functioning more constructively within her daily life. QMHP-C encouraged Ashiyah to present healthier habits and routine by challenging herself to make better choices and try new things. QMHP-C questioned Ashiyah on the statements she made about having difficulty expressing her feelings. Intervention: QMHP-C facilitated an activity on emotions to assist Ashiyah to be able to identify and positively express them. QMHP-C reviewed strategies targeted at helping Ashiyah to work through her difficulties expressing her anger. QMHP-C utilized age appropriate mood regulation skills and techniques, as a way to help Ashiyah’s destructive and critical demeanor, as well as to assist her with regaining and regulating her uncontrollable conduct. Intervention: QMHP-C encouraged Ashiyah to refrain from displaying negative behaviors towards her mom, to talk calmly and express herself without aggression. QMHP-C demonstrated and provided positive empathetic acceptance while utilizing talk therapy and active listening skills to enhance self-esteem and confidence. QMHP-C reemphasized with Ashiyah about the basics of treating others respectfully and focusing on treating others the way she would want to be treated. QMHP-C examined and evaluated Ashiyah responses to feedback. QMHP-C focused on Ashiyah’s ISP objectives and what she needs to do in order to achieve them. QMHP-C ended session with a small pep talk and encouraged Ashiyah to have a good rest of her day. Response: Ashiyah was reluctant and hesitate, as she remained mistrustful in regard to extending a friendly or respectful rapport back to the QMHP-C. Ashiyah acknowledged QMHP-C’s redirection after 3-4 prompts. Ashiyah stated okay and then began to calm down her actions. Ashiyah proceeded to lower her voice while calming down. Ashiyah appeared disinterested in regard to the information that was being provided to her by the QMHP-C in response to how her negativity affects herself and others around her. Ashiyah exhibited an unemotional and flat facial expression on her face, indicating she was not in the mood to process or reflect on her day. Ashiyah was resistance to applying what she learned to real life situations. Response: Ashiyah went back to her normal episodes and was not responsive to intervention. The interventions were noted unsuccessful, as it was difficult and challenging for Ashiyah to relax when being instructed by the use of calming techniques. Ashiyah displayed body language associated with anger, demonstrated by clenching her fists and face. Ashiyah had to be redirected in regard to the relaxation and calming imageries. Response: Ashiyah did not offer verbal communication. Therapy activity was viewed as ineffective and unproductive. Ashiyah displayed a disinterest in the interventions that were applied to her. Ashiyah expressed anger and frustration when she had to communicate directly with her mom without looking at QMHP-C for additional support.
